Because of our active care management system, HomeWell currently has a 0% rehospitalization rate for nearly a year! With our help, you can get back to stress-free living knowing that we’ve got everything under control.</p> <h3>Service Options</h3> <p>At HomeWell, we create a customized care plan for each client with a no-charge assessment. We also offer a Life Enhancing Activities Program (LEAP) to help ensure the emotional and physical well-being of clients with the implementation of an active lifestyle — something everyone can benefit from.</p> <p>HomeWell provides you with personal assistants that can fulfill all household duties from laundry and cleaning to meal preparation. Home health aides can also assist in bathing, toileting, feeding, and getting around in any way needed. Our services are many and can be adapted to your needs. Just let us know how we can help.</p>
