My Loved One is Now Sufferring After a 4 Month Stay at Clark Manor
Clark Manor Assisted Living occupies a "Grand and Nostalgic" manor style home in serene surroundings. The Owner/Manager and the Staff are extremely pleasant and present themselves as compassionate and competent individuals. Unfortunately, what my 88 year old loved one with mild to moderate dimentia & age-related and other out-patient managed medical conditions, experienced during her 4 month stay there (which ended 4 days ago) was quite the opposite of grand and serene. The care that she received was ANYTHING BUT compassionate and competent. Instead, It has proven to have been extremely damaging to her overall health status and has contributed greatly to and in some areas has been the single cause of significant decline. The Owner/Manager and the hands on staff (of which there was a high frequency of turn over, specifically 4 changes in a 2 month period) were very pleasant, but did not demonstrate adequate training or preparedness for the assistance that the facility had CONTRACTED to provide to my loved one. Though she did not have overwhelming needs, consistency of the care that she did need was important. Since the facility was populated at only a small percentage of its full capacity, my loved one had a private room and bathroom. I initially thought this was a great thing. But later began to notice that the staff was leaving the responsibility for the cleaning of her room and bathroom largely to her. Because she suffered from incontinence (she utilized incontenence briefs) and impaired mobility (she used a walker), her bathroom smelled bad most of the time, and the floor was frequently sticky with soap and urine residue and more. It was only cleaned by staff when I or other visitors requested the cleaning. The facility was also inconsistently heated. Frequently during visits I would observe that there was very little heat. My loved one and the other 2 residents would be draped in throws and comforters while huddled in front of the television. And I would be told that a repairman was on the way. The monthly fee would have been reasonable if the actual services that the facility claimed AND CONTRACTED to provide were actually rendered. Since they were not, I now look back and feel that the facility was not a good value at all. And MOST IMPORTANT...I later learned that the $2000.00 non-refundable initial deposit was WAY out of line with other home-based facilities of the same model as Clark Manor. The care services for my loved one were rendered as follows: 1. Her medications had been improperly administered by staff. This was evidenced by the condition and random pattern of dispensory of pre-packaged/pre-dated medication bubble packs provided to me upon her discharge. Upon arrival at her new facility, it was determined by medical staff there that her medication packs needed to be replaced immediately by her pharmacy so they could start from scratch. The dispensory pattern utilized at Clarke Manor could not be deciphered and certainly did not match the accompanying state required documentation of dispensory that was provided to them by Clarke Manor. Also, it was determined that medication orders had not been properly comprehended and therefore not properly followed. 2. Important medical information and reporting was frequently misplaced, ignored, or improperly interpreted or responded to by the Assisted Living Program manager. As a result, staff was not informed of and therefore did not carry out the neccessary corresponding care actions. 3. My loved one suffered a decline in overall skin integrity and specifically developed 3 areas of SERIOUS skin breakdown (skin ulcers) in her perineal area requiring treatment by a wound care physician due to lack of cleaning and bathing of the area and lack of positional change. The existence of these conditions was NEVER reported to me by the facility. I found out by chance, and also by the assessment which was conducted on the exact day of her discharge from Clark Manor by the State Delegated Nurse at her new facility. 4. Though denture adhesive was never applied to her dentures, they became stuck to her mouth with food debris due to lack of removal and cleaning. 5. While at Clark Manor, my loved one experienced a fall and minor injury treated by the facility's Delegate Nurse which was not reported to me until more than 3 weeks later. The dressing/bandaging of the injury was not visible due to clothing. Though vividly described in the beginning, I saw no evidence of Activities going on at the facility. A very large older model television with an impaired picture tube was the focus of activity everytime I visited. There was one lunch outing and one holiday party during my loved one's 4 month stay there. Overall, In spite of assessing my loved one with a moderate level of needs before her placement, then contracting to meet those needs, and to provide a clearly defined list of services, Clark Manor Assisted Living was neither inclined nor equipped to do as they contracted. And they in fact, DID NOT provide services as they contracted to do.
